Short Term Rehabilitation

The Rehabilitation Department at Schuyler Ridge welcomes you to our facility. We offer a progressive short -term rehabilitation program designed to assist patients with maximizing their independence with a primary goal of returning to the community.

We provide a multidisciplinary approach in a dynamic, positive environment. Our team of physicians, nurses, therapist, social workers and primary care aides, are dedicated to meeting all physical as well as psychosocial needs of our patients. We are committed to providing the highest quality of care with respect, dignity and compassion. Our highly skilled professionals work as a team collaborating closely with patients and family for the most comprehensive individualized plan of care.

Our rehabilitation staff will evaluate all patients upon admission and establish a treatment program tailored to meet individual needs. Focus of treatment will be on functional mobility, self care skills and patient and family education. Our team will work with patients and family to establish the safest, most functional level of discharge possible. In addition we provide patients and their families with ongoing support and resources to assure that return to the community is as successful as possible.

Referrals to our short-term rehab program are accepted from physicians, hospital discharge planners and home health agencies.
